What is Form T-12 (working time sheet)

Based on the T-12 form, introduced into document management practice by the State Statistics Committee in Resolution No. 1 dated January 5, 2004, a time sheet is generated, which is intended for use for the following purposes:

  • keeping records of employee visits to their place of work in accordance with the established schedule;
  • determining indicators for calculating employee salaries;
  • generation of certain statistical data on personnel (for example, sent to Rosstat or research agencies).

It should be noted that the form in question must be filled out manually (on a PC using Word or a similar program, or printed out with a ballpoint pen). And the time sheet, intended to be filled out automatically when access control systems are activated, is compiled on the basis of another Goskomstat form - T-13. The selected form of the 2017 time sheet must be indicated in the accounting policy.

When is it supposed to use the work time sheet form 0504421 according to OKUD

The work time sheet form corresponding to OKUD number 0504421 (T-12 has a very similar name to it, so sometimes confusion can arise when applying a particular document) was introduced into business circulation by the Ministry of Finance of the Russian Federation, which issued order No. 52n dated March 30, 2015. This document is used for the same purposes as T-12, but is subject to use in government agencies.

The time sheet corresponding to form 0504421 also has a fairly similar structure to form T-12. Therefore, for an employee of the HR department, as a rule, there is no problem adapting to a document approved by the Ministry of Finance if he is accustomed to using the form from Goskomstat, and vice versa.

Is the use of time sheets f. allowed in private companies? 0504421

The use of timesheets in form 0504421 is not prohibited for private companies. The fact is that since 01/01/2013, companies that are not directly required by law to use specific unified forms of primary sources are allowed to use any others. Therefore, a private company has the right to use, in order to monitor the time spent at work by hired employees, a time sheet on the form T-12, form 0504421, or another form developed independently. One way or another, the company must have such a document - due to the fact that, in accordance with the provisions of Art. 91 of the Labor Code of the Russian Federation, each employer must monitor the time spent at work by hired employees.

How are vacations indicated on the time sheet?

Unified forms for keeping records of employee visits at Russian enterprises T-12 and T-13 involve reflecting information about vacations using the codes given on the title page of form T-12.

These codes are presented in two varieties: alphabetic and digital. The use of both is equivalent. Moreover, the organization’s local regulations may also stipulate a certain mixed application option or involve the use of completely different codes. The employer also has the right to independently develop and use a report card form with symbols in it.

Primary and additional leave

When an employee goes on regular paid leave, and the company uses the T-12 or T-13 form, the letter code OT or digital code 09 is recorded in the accounting table, which is given in the report card, for each day of the employee’s vacation.

If the rest is additional, then a different code is entered: OD (10).

Study and unpaid leave (administrative leave)

In the provisions of Art. 128 of the Labor Code of the Russian Federation, leave at the expense of the employee is divided into 2 types:

  • provided by the employer voluntarily at the request of the employee - in this case, the DO code (16) is reflected in the timesheet;
  • mandatory provided by the employer at the request of the employee - OZ code (17) is used.

Study leave also has 2 types according to labor legislation (Article 173 of the Labor Code of the Russian Federation):

  • leave for study with preservation of earnings - reflected using code U (11);
  • unpaid leave for passing entrance exams, sessions, state exams - is recorded in the report card using the UD code (13).

Maternity and child care leave

Maternity leave provided in accordance with Art. 255 of the Labor Code of the Russian Federation, is reflected in the report card using code P (14). Child care leave provided under Art. 256 of the Labor Code of the Russian Federation, is fixed using the coolant code (15).

A scenario is possible in which an employee on maternity leave will work part-time. If this is so, then when recording her visits in the report card, the “double” code I (01) and OZH (15) will be used. These codes can be specified in one cell of the timesheet using the “/” symbol (for example, Я/Ож or 01/15), or you can add an additional line to the timesheet form.

Theoretically, there can be three codes in one cell. For example, if an employee agreed to go on a business trip and worked a day off there. In this case, the table will write: K/RV/Coolant (06/03/15).

What to put on your report card if your vacation falls on a holiday

In accordance with Art. 120 of the Labor Code of the Russian Federation, non-working holidays falling during the period of the annual main or annual additional paid leave are not included in the number of calendar days of leave and are not taken into account.

Holidays established by regional legislation are also excluded from vacation (see Resolution of the Presidium of the Supreme Court of the Russian Federation dated December 21, 2011 No. 20-ПВ11).

It follows that holidays falling on vacation in the report card must be designated by code “B” or 26.

What is this document and what is it for?

By Decree of the State Statistics Committee No. 1 of January 5, 2004, a special form was approved, which is administered by the personnel department and accounting department.

This form allows you to do the following:

  • keep records of hours worked, as well as reflect information about hours not worked by the employee;
  • monitor compliance with work discipline, reflect attendance/absence, facts of being late for work;
  • have sufficient information about the amount of work performed by the employee in order to further correctly calculate wages and enter information for statistics.

Forms No. T-12 and T-13 serve as the basis for the accountant to accrue wages and additional compensation to each employee. For the HR department, the report card will allow you to monitor the attendance of employees, and if violations are detected, impose penalties on the offending employee.

Tax legislation provides for the right of an employee upon dismissal to request a time sheet along with the issuance of other personal documents, including a work book. This provision is enshrined in Art. 84.1 NK.

Enterprise managers should be aware that since 2013, unified forms have been abolished. Despite the abolition of strictly established forms, the obligation to keep records of employees’ work time in the organization remains in accordance with Part 4 of Art. 91 TK. Thus, the enterprise has the opportunity to make adjustments to the design of the document for recording the time worked by employees.

Since the form introduced by Goskomstat is convenient for entering data and further processing, many organizations continue to use it.

Who is responsible for maintaining the document?

According to the Instructions for the use and completion of primary accounting documentation in force in 2019, the forms are prepared by a separate employee who has been assigned this action. The document must bear the certifying signature of the manager and the personnel officer.

A separate position for accounting at the enterprise is not required; this work can be performed by any employee who is given authority to do so by management. This occurs by issuing an order appointing an employee of an established position responsible for recording work time.

In addition, it will be necessary to introduce a separate clause into the employment contract on granting such powers to the employee. If this is not done, management does not have the right to demand that the employee perform accounting duties.

The appointed official enters the data within a month, and then submits the document to the head of the department and the employee of the personnel department for signing. The personnel officer checks the data, takes the information necessary to perform personnel work from the provided report, then signs the form and passes it on to the accounting department for further work.

If the organization is not large, time tracking is made the responsibility of a personnel employee, who then gives the document to the accountant to calculate wages.

Rules for filling out a time sheet

The productivity of the accountant and HR officer will depend on how convenient it is to work with the report, and the correct reflection of information about the time actually spent at the workplace will make it possible to make correct calculations of payment to the employee, which is beneficial for both parties - the employee and his employer.

On the one hand, the employer has the right to make a deduction if the actual time worked does not correspond to the time established in the contract. On the other hand, a watchman who went to work on a day off has the right to demand additional compensation for the amount of work performed.

The document is an act on the work actually performed by each employee and on the working regime in force at the enterprise, recording any deviations from the established work schedule.

The timesheet is required for the operation of the following organizational structures:

Following simple rules, an employee can quickly and competently draw up a report on the time actually worked by the company’s employees during the month:

  1. The report card is filled out in a single copy.
  2. You can fill out the sample form manually, or enter information via a computer.
  3. In order for an employee to have the right to carry out accounting, the head of the enterprise must issue an appropriate order to vest the employee with the necessary powers. This responsibility should be included in the job description.
  4. Preparation of the timesheet begins on the eve of the reporting month.
  5. The title page contains information about the full name of the enterprise, division, as well as the start date of accounting. At the end of the month and the timesheet is closed, the end date of the report is entered.
  6. The following sheet is designed in the form of a table with information about employees filled out. It is necessary to ensure that the surnames and first names of employees are correctly indicated, avoiding the possibility of confusion among employees with the same surname.
  7. For each employee, information about his personal file number is entered.
  8. Each column of the timesheet corresponds to a specific day of the month.
  9. The cells opposite the names contain information about the hours worked, the number of shifts, and if there are weekends and holidays, the corresponding marks are entered.
  10. Information about the work actually performed is entered every day, based on the official documents provided (sick leave certificates, orders, statements, management orders).
  11. Cases of tardiness and absenteeism are also subject to reflection in the form of letter and numeric codes.

The document is considered valid if it is signed by the authorities. Responsibility for the correct entry of information rests entirely with the designated employee.

Features for budget organizations

In enterprises operating on a budget, a different form of accounting has been introduced, approved in the form of form No. 0504421. The form differs from the general forms T-12 and T-13, due to the reflection of the specifics of the work of state employees. New codes are introduced in the table to be filled out: study days off, replacement in extended day groups, study leaves.

The title and tabular parts have some differences:

  • the title page, in addition to information about the name of the organization, contains a digital code;
  • if there are no changes, the column is marked “0”;
  • if adjustments were made, the serial number of the changes is indicated;
  • columns 20, 37 contain information on intermediate and monthly totals of working time;
  • no additional calculations are made in the form.

Otherwise, the rules for filling out this form differ little from the standardized forms.

Filling out a working time sheet according to the sample involves the use of digital and alphabetic designations, with a mandatory decoding on the title page of the report. Since in some cases the established codes are not enough, management should issue a separate order approving additional designations (for example, breaks for breastfeeding).

When checking the sample filling, you should pay attention to the most common errors when filling out:

  • Only the last name with initials is entered, without position;
  • indicating a holiday non-working day as a working day;
  • indicating the duration of the pre-holiday day as full (“8” instead of “7”).

When recording the work of a part-time worker, the exact duration of work must be noted. The hours and minutes spent by such an employee on performing his duties are indicated in the bottom line in columns 4, 6 (on form T-12) or the 2nd and 4th line in column 4 (on form T-13). In case of internal part-time work, the duration of work is distributed according to positions.

If an employee is on a business trip, the letter code “K” or the numeric code “06” is entered on the time sheet without indicating the hours of work.

Penalties for incorrect handling

Proper execution of the report card is controlled by supervisory government agencies. If violations are detected during any of the audits (financial or tax), the organization will be punished.

The most frequently identified inconsistencies are:

  • there is a discrepancy between the data in the accounting sheet and the primary reporting;
  • salary data does not correspond to calculations in payment documents;
  • codes were entered with errors, incorrect codes were used without taking into account overtime and overtime;
  • Incorrect calculation of sick leave benefits.

If a violation is established, the company may be fined up to 5 thousand rubles. A much more serious punishment awaits if the mandatory time sheet was not kept at all. The amount of the fine can be up to 50 thousand rubles, levied on the managerial person on the basis of Art. 5.27 Code of Administrative Offences.

The time sheet is intended for entering information about the time actually worked by employees of organizations. It must be said that the time sheet form is not strictly mandatory - in principle, it can be arbitrary, that is, each enterprise is free to use its own time sheet form if such a need arises. However, the form was developed and recommended for use by the State Statistics Committee of the Russian Federation and is preferable.

Who fills out the time sheet?

The form is filled out either by an employee of the HR department, or by the head of a structural unit, or by a timekeeper specially hired for this function. Based on the information entered into it, the accounting department specialists calculate wages and other payments to the organization’s employees. In fact, the time sheet is one of the most important accounting documents. And while small companies can easily do without it, large enterprises are required to keep such timesheets.

Depending on the personnel records system adopted at the enterprise, a timesheet can be generated either one for all employees of the organization, or maintained separately in each department.

The time sheet is a regular document, that is, a new copy must be compiled every month, so the serial number of the time sheet will be equal to the serial number of the month in which it was created. The timesheet preparation period covers all days of the month.

You can fill out the timesheet either electronically or in writing. However, after entering all the necessary information, it will still have to be printed for the signatures of the responsible persons.

Form T-13. Shape Features

Let's start with the T-13 form, which is now used much more often for maintaining time sheets.

The unified form T-13 or electronic time sheet is well known to HR department employees. This is not the only way, but it is definitely the most standard way to account for hours worked. If you keep records manually, you should use Form T-12.

Timesheets are a common tool for tracking employee attendance. Form T-13 allows you to record in detail the reasons for absence from work, including student leave during the session, advanced training and several types of disability leave. The period for which the document is completed may be less than 31 days.

A completed T-13 is the basis for calculating wages.

Format for filling out a working time sheet in T-13

Unlike arbitrary tables of similar content, T-13 contains data about the enterprise, including the form of ownership and OKPO. The document number is entered in accordance with the internal requirements for maintaining timesheets.

The department name is also shown at the top. It must be remembered that the head of this department (even if filling out the time sheet is not his or her responsibility) must sign the completed form.

The order of employees is determined by the decision of the person in charge. Most often, alphabetical sorting is found, as in our example, but the option of arranging by personnel number is possible (column 3).

In column 4 we put marks by day:

Under the I mark we put the number of hours worked that day. In column 5 we summarize the number of I in the line and the number of hours. We get 4 values ​​for 2 halves of the month. In column 6 we sum up the values ​​and get the final figure for the work for the month.

The number of hours for B, OT, K, B and other cases is not indicated in the fourth column. For this there are columns 10-13.

Accounting for sick leave, vacations or absences for other reasons

Designation codes may be different (for example, numeric). There is no specific format required by law.

The notation X shows that we are not taking this day into account: for convenience, the month is divided into two lines with unequal values. For months with 30 days (for example, November, the column will look like this (for convenience, the “non-existent” 31st number is highlighted in red):

Columns 7-9 indicate the payment code, number of days and type of charges. Our example uses the following codes:

  • 2000 - Common workday,
  • 2300 – sick leave (disability benefit),
  • 2012 - vacation.

Alternative solution

Some enterprises approve a slightly simplified version of the time sheet without detailing the reasons for omissions. Column 4 indicates only 2 codes:

  • I- working day,
  • N- unworked day.

This method may be inconvenient because it does not record sick leave.

Special cases

  2. Depends on the position of the enterprise. These days can be counted as working days (I), or as advanced training (PC). Pay rates may also vary.

Form T-12

First of all, as in any other personnel records document, you first need to enter the details of the organization into the timesheet: its full name indicating the OKPO code (must be taken from the registration documents), organizational and legal status (IP, LLC, CJSC, JSC), as well as the structural unit (department) for which this timesheet is maintained (if necessary).

Then you need to enter the document number for internal document flow in the appropriate column, and also indicate the reporting period that this timesheet takes into account.

Numerical and alphabetic codes in the time sheet

This part of the timesheet includes the alphabetic and numeric codes used to fill out the necessary information on employees, as well as their decoding. They must be entered in the main part of the timesheet in order to briefly and clearly reflect the amount of time actually spent by one or another employee at the workplace, as well as the reasons for his absence from work. If HR department specialists need to enter some additional codes into this timesheet form, they can be developed independently and entered into this table.

Working time recording in T-12

This section in the timesheet is the main one - it is where working time is kept track of. First, you need to enter the employee’s serial number in the first column of this section, then in the second - his full name (preferably his full name and patronymic to avoid confusion and errors). In the third column you need to insert the employee’s personnel number assigned to him during employment (it is individual and never repeated).

For each employee, the timesheet has two lines - they contain encrypted information about presence or absence at the workplace on each calendar day of the month. In addition, it is necessary to immediately indicate the reason for absence from work, if one has been established.

The reason is indicated in the top line opposite the employee’s full name, and in the bottom line the number of hours actually worked, and if the employee did not appear at the workplace, the bottom cell can be left empty.

The next step is to calculate the total number of hours and days actually worked for two-week periods, and at the end of the table - the result of the calculations for the month.


In this case, you should carefully monitor that the total number of calendar days in a month coincides with the amount of working days, weekends and holidays indicated for each employee.

It should be said that sometimes those responsible for filling out the time sheet only enter information that relates to the days when the employee was absent from the workplace. However, this option may lead to personnel and accounting errors, so it is not advisable to use it.

Date and signatures of responsible persons

After the time sheet is filled out, the employee responsible for it must indicate his position, as well as put a signature in the appropriate cells, which must be deciphered. The report card must also be approved by the head of the structural unit or the director of the enterprise - also indicating the position and signature with a transcript. When filling out the timesheet, do not forget to indicate the name of the organization and structural unit.

In the “Reporting period” cell we enter the beginning and end of the month for which the timesheet is maintained.

The top line of the table contains an alphabetic or numeric working time code for each day of the month (appearance, business trip, etc.). The number of hours for them is recorded at the bottom. For example, if an employee worked a full shift on July 3, 2014, then in the timesheet for July, in the cell with the number 3 opposite the employee’s last name, the code “I” and 8 working hours are entered. If an employee is sick that day, then it is necessary to enter “B”, and since he did not have working hours, the bottom cell is not filled in or is entered as 0.

At the end of the timesheet, we make sure that there is a signature of the person responsible for filling out the timesheet, as well as the signature of the head of the structural unit and the HR employee.

Non-standard situations when filling out a timesheet

1. The employee fell ill while on vacation.

After the vacation, the employee returned to work and provided sick leave for the period of illness. In the report card, instead of leave (OT), a temporary disability code (B) is entered from the first day of illness, and the leave itself is extended for the period of illness.

There are cases when an employee continues to be sick even after the end of his vacation, having warned the employer. Since the employee’s words are not the basis for filling out a time sheet, therefore, after vacation, we set absences for unknown reasons (NU). After the employee provides sick leave, we correct the period to code B.

2. The employee had holidays during the vacation period.

The employee, for example, was on vacation from June 1 to June 29, 2014. In the working time sheet, we enter the code OT for the main vacation period, while June 12 is marked with code B (non-working holiday), since this day is not included in the paid vacation period.

When concluding labor contracts with employees, a business entity must organize the recording of their work time. For these purposes, a time sheet can be used, which is opened every month, and in it the person in charge reflects the employees’ working time, their vacations, sick leave and other types of absence from work. According to the data contained in this document, salaries are subsequently calculated.

Legislation requires the administration of an organization or individual entrepreneur to organize and maintain records of periods of work for each employee. Filling out the work time sheet can be carried out by the responsible person, who is determined by order of management.

How is an employee's working time calculated?

The law establishes two types of standards - a six-day work week (36 hours) and a five-day work week (40 hours). That is, workers can work five days with an eight-hour work day, or six days with a six-hour day. Violation of them is allowed in rare cases - with summarized accounting or an irregular schedule.

In the first case, the standards are applied in a larger period of time, for example, a quarter, half a year, etc. It turns out that in a shorter period of work, the fact may not correspond to the current standards, but it should not exceed the standards in the selected larger time periods.

For some workers, a reduced daily or weekly rate may apply. How exactly you need to take into account the working time of employees must be recorded in. The timesheet must also reflect all the time when the employee did not work, but was registered at the enterprise.

Such periods may include:

  • Sick leave.
  • Downtime, etc.

The timesheet opens at the beginning of the month, and at the end of the month it is closed. In the middle of the month, the responsible person summarizes the interim results, reflecting the data for the first part of the work time. The document is signed by the head of the department and submitted to the HR department for verification. This is then sent to the accounting department for payroll calculation.

Attention! The time sheet for 2017 can be, as in previous periods, of two types - form T-12 and form T-13. The first involves not only recording work time, but also the possibility of calculating wages. Form T-13 is used only to record work time; other documents are used to calculate wages.

Attention! If the reason for absence is unknown, then the letter code “NN” must be entered on the report card. In the future, this code will be refined. If the employee was sick during this period, the code is corrected to “B”. In the case where there are no supporting documents, then instead of the code “NN” the code “PR” is entered.

Holidays fell during vacation

According to the Labor Code, if holidays fall during the vacation period, they are not included in the calculation of calendar days.

When an employee is granted annual leave, during its period, weekends are not noted in the time sheet, because they are included in the number of calendar days - in their place is the letter code “OT” or the digital designation 09 for annual leave, as well as the code OD or designation 10 - for additional leave.

Attention! Non-working holidays are not included in the number of calendar days. Therefore, in the timesheet such days must be designated by the letter code “B” or the number 26.

An employee fell ill while on vacation

If an employee falls ill while on vacation, then to confirm this fact he must be provided with a properly issued sick leave. As a result, rest days must be extended while on sick leave, or postponed to another time.

Initially, the vacation time should be marked in the timesheet with the letter code “OT” or the digital designation 09. After the sick leave is provided, the timesheet must be adjusted - for sick days, instead of the previous designation, the code “B” or the digital designation 19 is written.

The business trip fell on the weekend

According to the letter from the Ministry of Labor, all days of a business trip must be noted on the timesheet, even if they fall on weekends. To do this, you need to use the designations in the timesheet - a special letter code “K” or the digital designation 06. In this case, you do not need to enter the number of hours.

If, while on a business trip, the employee worked on weekends, then in the timesheet they are marked with the code “РВ” - work on weekends, or with the digital designation 03. The number of hours of work must be entered only in one case - when the company management gave the employee specific instructions , how many hours of the day off he needs to devote to work.
